Please register for the Forum using the form available for download below:

Important information you need to know beofre you complete the Registration Form.

  • The Registration Form is also a tax invoice
  • For your convenience you can register for all three Fora in a particular year, or register more than one person from your organisation, all on the same form
  • Please complete all details on the form including the Method of Payment section
  • Please ensure you return the form to the contact person on the form either by email, fax or mail.
