To register now for one or more of the 2009 Fora please use the Tax Invoice Registration Form below and indicate the dates of the Fora and the number of people you are registering for.

Before you complete the Tax Invoice Registration Form, please read this information carefully:

• The Registartion Form is also a Tax Invoice, therefore, no other invoice or receipt is issued
• No order number is required
• For your convienience you can register for all three Fora in a particular year, or register more than one person from your organisation, all on the same form
• Please complete all detail on teh form including Method of Payment section
• Please ensure you reurn the form to the contact person on teh form either by email, fax or mail.
